7. Safety Officer Certificate (refer to D.O. 198 series of 2018)

8. Occupational Health Personnel (refer to D.O. 198 series of 2018)

9. Certificate of Inspection and Testing of Construction Heavy Equipment (CHE) (if

applicable)

10. Skills Certificate of Construction Heavy Equipment Operators issued by TESDA
