(Read this Product Disclosure Sheet before you decide to PRULady non-participating insurance plan
take out the PRULady. Be sure to also read the general
terms and conditions.) 24/12/2023

1. What is this product about?


PRULady is a non-participating insurance plan that provides coverage for 41 years. It pays a lump sum benefit if you die /
suffer Total and Permanent Disability (TPD) or are diagnosed with any Female Related Illnesses. In addition, this plan
provides comprehensive coverage such as Recovery Benefit, Wellness Care Benefit, Fertility Care Benefit, Pregnancy Care
Benefit, Baby Care Benefit as well as cash rewards to celebrate the important milestones of your life. Furthermore, it also
refunds the total premium paid at maturity of the policy. Please refer to the table of benefits below for details.
2. What are the covers / benefits provided?
The table below summarises the benefits that you have selected. Please refer to the relevant Appendices for more information
about the benefits of this plan.

Basic Sum Assured of this plan is RM50,000.

Death Benefit or Total and Permanent Disability (TPD) Benefit


Death Benefit The higher of
• 100% of Basic Sum Assured; or
• Total premium paid.

Total and Permanent Disability (TPD) Benefit The higher of


• 100% of Basic Sum Assured; or
• Total premium paid.

Female Related Illnesses Benefit


a. Female Cancer The higher of
• Breast; • 100% of Basic Sum Assured; or
• Cervix Uteri; • Total premium paid.
• Fallopian Tube;
• Ovary;
• Uterus; or
• Vagina/ Vulva.

b. Systemic Lupus Erythematosus with Severe Kidney


Complications.

Carcinoma-in-situ 50% of Basic Sum Assured.


• Breast;
• Cervix Uteri;
• Fallopian Tube;
• Ovary; Notes:
• Uterus; or • This benefit is claimable once only.
• Vagina/ Vulva. • Payment of this benefit will reduce the benefit
payable under Death, TPD or Female Related
Illnesses. Thereafter, the coverage amount for
Death, TPD or Female Related Illnesses will be
restored to the original Sum Assured before the
payment of this benefit, 6 months after the
diagnosis date.

Recovery and Wellness Care Benefit


Recovery Benefit 30% of Basic Sum Assured for each claim.
a. Surgical Removal of Reproductive Organ due to
Carcinoma-In-Situ, Cancer or Accident;
b. Breast Lumpectomy or Mastectomy due to Carcinoma-In-
Situ, Cancer or Accident;
c. Breast Reconstructive Surgery following a Mastectomy due
to Carcinoma-In-Situ, Cancer or Accident;
d. Facial Reconstructive Surgery due to Cancer or Accident;
e. Skin Grafting due to Burn or Skin Cancer;
f. Osteoporotic Fracture; or Note: Only 1 claim is allowed for each condition, up to a
g. Rheumatoid Arthritis. maximum of 2 conditions.

Wellness Care Benefit Reimbursement up to RM250 per visit, subject to


RM5,000 per life.
a. Psychotherapy Treatment/ Psychiatric Visit due to
• Depressive Disorder; and/ or
• Anxiety Disorder.

b. Dermatologist Visit due to Severe Acne Diagnosis.

Fertility, Pregnancy and Baby Care Benefit


Fertility Care Benefit Reimbursement up to 20% of Basic Sum Assured,
subject to RM15,000 per life.
a. Oocyte Cryopreservation;
b. In-Vitro Fertilization (IVF) Treatment; or Notes:
c. Intrauterine Insemination (IUI) Treatment. • Total claim payable for IUI treatment is up to 10%
of Basic Sum Assured, subject to RM7,500 per life.
• This benefit will cease when the Life Assured
attains age 45.

Pregnancy Care Benefit 10% of Basic Sum Assured upon diagnosis of any one
of the 12 pregnancy complications covered.
a. Disseminated Intravascular Coagulation (after 7 months of
pregnancy);
b. Ectopic Pregnancy;
c. Hydatidiform Mole;
d. Eclampsia;
e. Death of Foetus (after 13th week of pregnancy);
f. Death of the Life Assured’s Child (within 30 days of birth);
g. Abruptio Placentae;
h. Amniotic Fluid Embolism;
i. Acute Fatty Liver of Pregnancy; Notes:
j. Postpartum Hemorrhage Requiring Hysterectomy; • This benefit is claimable once only.
k. Pulmonary Embolism of Pregnancy; or • This benefit will cease when the Life Assured
l. Placenta Increta/ Percreta. attains age 50.

Baby Care Benefit 10% of Basic Sum Assured upon diagnosis of any one
of the 8 congenital anomalies covered.
a. Ventricular Septal Defect requiring surgery;
b. Atrial Septal Defect requiring surgery;
c. Down’s Syndrome;
d. Spina Bifida; Notes:
e. Tetralogy of Fallot; • This benefit is claimable once only.
f. Cleft Lip and/or Cleft Palate requiring surgery;
• This benefit will cease when the Life Assured
g. Transposition of the Great Vessels; or
attains age 50.
h. Patent Ductus Arteriosus requiring surgery.

In addition to the benefits above, you will receive the following cash rewards under this plan.
Cash Rewards
Life Celebration Benefit RM1,500, which is 3% of Basic Sum Assured is payable
if any one of the covered events is achieved, up to a
a. Job Promotion; maximum of 3 events.
b. Doubling of Annual Income;
c. Owning a Car;
d. Owning a Property;
e. Travelling to 10 different Countries;
f. Marriage;
g. Having a Child (Pregnancy after 13 weeks/ Legal
Adoption);
h. Every 10th Wedding Anniversary; Notes:
i. Retirement (when the Life Assured attains age 55); or • Each event is claimable once except for Having a
j. Completion of 3 Fitness Goals (with certification) from any Child/ Wedding Anniversary which is claimable up

• Completed half/ full marathon; • For Retirement, 9% of Basic Sum Assured is

Golden Cash Rewards RM1,950.00, which is 5% of total premium paid (i.e. total
premium payable up to end of premium payment term)
is payable upon Policy Anniversary of Life Assured’s
Age Next Birthday (ANB) 60 and 65 respectively.

Money Back Benefit RM39,000.00, which is the refund of total premium paid
is payable upon maturity.

Total Cash Rewards Up to RM47,400.00

Notes:
a) This policy will be terminated after you have made a claim on Death, TPD, Female Cancer or Systemic Lupus
Erythematosus With Severe Kidney Complications.
b) The total premium paid is excluding extra premium charged for sub-standard life and taxes (if applicable) and may vary
by the change of payment frequency.
c) Upon the claims on Female Cancer Benefit, if you have not made the maximum of 2 claims under Recovery Benefit, we
shall pay you the Recovery Benefit together with the Female Cancer Benefit. Please refer to the policy contract for the
detailed terms and conditions.
d) For "Surgical Removal of Reproductive Organ", "Breast Lumpectomy or Mastectomy" and "Breast Reconstructive Surgery
following a Mastectomy" due to Carcinoma-In-Situ/ Cancer, only the sites covered under Female Related Illnesses Benefit
are applicable.

3. How much premium do I have to pay?


The total premium that you have to pay and the policy terms may vary depending on the underwriting requirements of the
insurance company:
• The estimated total premium that you have to pay: RM125.00 monthly.
• Premium duration: 26 years.

Premiums for this policy are not guaranteed. We reserve the right to:
• Revise the premium during premium payment period; and/ or
• Revise the premium and request for any additional premium to be payable after premium payment period during the policy
term,
by giving at least 90-day prior notice before taking effect on the following Policy Anniversary.

In the event of premium revision or request for any additional premium to be payable during the policy term, we will also
provide you the option to pay the same premium or not to pay the additional premium requested, if any, but there will be a
reduction to the amount of benefit and/or surrender value (whichever is applicable) under your policy.

In the event of taking up any of the above options, we shall notify you the changes. The new amount of benefits and/or
surrender value (whichever is applicable) shall take effect on the next Policy Anniversary from the date we receive your
notification.

4. What are the fees and charges I have to pay?


• Taxes at the prevailing rate may be charged on any of the premiums, or any other payments due (where applicable)
under this policy.
• For commissions paid to the agent/wealth planner, please refer to the Summary Illustration.

5. What are some of the key terms and conditions that I should be aware of?
• Importance of disclosure – you must disclose all material facts such as medical condition and state your age correctly
when answering any question asked by us. If the insurance policy is intended wholly for your personal purposes, you
must take reasonable care to disclose any facts that you know to be relevant to us and not to mislead us. Your duty
of disclosure continues until the policy is issued.

• Free-look period – you may cancel your policy by returning the policy within 15 days after the policy has been
delivered to you. The premiums that you have paid (less any medical fee incurred, if any) will be refunded to you.

• Grace period – you have a one-month grace period from each premium due date to pay the premium for this policy.
The policy will remain in force during the grace period.

• Nomination – you are advised to name a nominee(s) for your insurance application to ensure a smooth settlement of
claims. You should also ensure that the nominee(s) is aware of the policy that you have purchased.

• Claims – please refer to the policy contract to understand further on the claims requirements.

• Policy lapse – if you do not pay your premiums within the grace period, your policy may lapse unless your policy has
acquired cash value. In this case, we will automatically advance premiums on the security of the cash value under the
Automatic Premium Loan Scheme. You will continue to enjoy the benefits until the premium loan plus interest is greater
than the cash value. If your policy has lapsed, you may reinstate the policy subject to terms and conditions.

• Waiting period and survival period – we shall not pay any of the benefits if the condition or illness first happens or
manifests symptoms, or the events which leads to diagnosis become clear within the waiting period as stated below
from the effective date of the policy or revival date of the policy, whichever is later, and the survival period as stated
below from the Life Assured being diagnosed with covered conditions.

• The policy must be continuously in force for at least 1 year,
and
i) you have paid at least 2 full years of premiums, before the
first claim;
Life Celebration Benefit Nil
ii) you have paid at least 4 full years of premiums, before the
second claim; and
iii) you have paid at least 6 full years of premiums, before the
third claim.

Note: This list is non-exhaustive. Please refer to the policy contract for the terms and conditions under this policy.

6. What are the major exclusions under this policy?


• Suicide – if death was due to suicide within one year from the effective date of policy or the date of policy revival,
whichever is later, premiums paid (excluding taxes, if any) will be refunded without interest.
• Total and Permanent Disability (TPD) benefit is not payable if the disability is directly or indirectly caused by:
(a) any pre-existing conditions;
(b) any attempted suicide or self-inflicted injury whether attempted/inflicted while sane or insane;
(c) any traveling in an aircraft other than as a pilot or a member of a crew or a fare paying passenger in a commercial
aircraft licensed for passenger service on scheduled flights over established routes only; or
(d) any participation in any aerial sporting activities such as hang-gliding, ballooning, parachuting, sky-diving, bungee
jumping and other such similar activities.
• Female Related Illnesses Benefit, Recovery Benefit, Wellness Care Benefit, Fertility Care Benefit, Pregnancy Care
Benefit and Baby Care Benefit are not payable if any condition, illness, injury or event which is caused by or in
connection with any of the following:
(a) any pre-existing conditions;
(b) any self-inflicted injury or self-inflicted illness, whether inflicted while sane or insane;
(c) any injury or illness caused by war, invasion, act of foreign enemy hostilities, (whether war be declared or not)
civil war, rebellion, revolution, insurrection, military or usurped power, direct participation in a riot, strike or civil
commotion;
(d) the existence of Acquired Immune Deficiency Syndrome (AIDS) or by the presence of any Human Immuno-
deficiency Virus (HIV) infection;
(e) any surgery or treatment you have chosen to have rather than which is Medically Necessary (whether cosmetic,
plastic or otherwise) unless we cover it under this Policy;
(f) any act or omission that contravenes the law, or resisting arrest;
(g) alcohol or substance abuse or addiction;
(h) injuries or sickness arising from professional sports, racing of any kind, scuba-diving, aerial sport activities such
as hang-gliding, ballooning, and any other dangerous activities or sports, unless we agree differently in a special
endorsement; or
(i) nuclear fusion, nuclear fission, nuclear waste or any radioactive or ionizing radiation.

• In addition to the above, Pregnancy Care Benefit and Baby Care Benefit are not payable if any pregnancy
complications or congenital anomalies covered in connection with any of the following:
(a) the Life Assured choosing to have a termination of pregnancy other than for medical reasons; or
(b) the pregnancy results from fertility treatment, including In-Vitro Fertilization, except if such treatment is due to the
conditions specified under Fertility Care Benefit – IVF/ IUI Treatment.

7. Can I cancel my policy?


Buying a life policy is a long-term financial commitment. If you do not pay your premiums within the grace period, your policy
may lapse unless your policy has acquired cash value. The cash amount that the insurance company will pay you when you
cancel the policy before the maturity period will be much less than the total amount of premium that you have paid.

8. What do I need to do if there are changes to my contact details?


It is important that you inform us of any change in your contact details to ensure that all correspondences reach you in a
timely manner.
