1. This assignment shall be accomplished per group designated by the facilitator.

2. Based on your identified hazard to plan for, determine the needs that will arise and identify
the Response Clusters that will address them. For this purpose, accomplish CP Form 5 in
page 2.

3. Select one (1) Response Cluster from CP Form 5, then undertake Cluster Planning by
accomplishing the following forms:

a. CP Form 6 (see page 3)

b. CP Form 7 (see page 4)
c. CP Form 8 (see page 8)

Refer to CP Guidebook for detailed guide in filling-up the form(s).

4. Once accomplished, save the file and submit assignment via Training Information
Management System (IMS). Upload the assignment using the filename template
<Surname>_<Assignment Number> (example: Dela Cruz_Assignment3).

Deadline is not later than 1600H of this day.
