Breakdown of proposed production activity

Please provide details of dates, times and locations where each activity will occur. State whether you have obtained the permission of property owners to
work there and provide details. Remember, if you are filming in public spaces and parks within the city of Bristol you will need to seek
permission from the Bristol Film Office. If your crew is small and you are using only hand-held equipment this may not be necessary.

Persons at Risk of Injury or Harm

Think about the people who might be at risk of harm as a result of your proposed activity. Remember that you have a legal obligation to be
particularly concerned about children, the elderly and those who may be more vulnerable to certain hazards (such as pregnant women).

Yourself and other crew members / Members of the public and Passers by /

Important - High Risk Activities

Remember that certain film activities are High Risk, these include Working with Vehicles or near Roads, Working at
Height, Working on or near Water, Working at Night, Working with Mains Electricity and Filming with Weapons.
If any of the above are to take place they will need to be fully assessed with robust control measures put in place.

The Production Risk Assessment

The key elements of a Risk Assessment are the following;

 The identification of Hazards.
 To decide who might be harmed and how.
 To consider how severe that harm might be.
 To evaluate the risks and decide on precautions (control measures).
 To record your findings in the form of a written Risk Assessment (below).
 To review the Risk Assessment and update it if anything significant changes.

Hazards Identified and Risks Arising Risk Assessment & Precautions Required

Identify and list what hazards could cause significant harm, Evaluate the risks and decide if any existing precautions/controls are
how and to whom. This includes anybody who may be sufficient or more needs to be done. Take into account information from
present and affected by your activity, not only your team all available authoritative sources
(e.g. other workers, members of the public, children,

Light can be harmful Me the tracer looking into the projector Avoid standing in front of projector and looking directly at projector

fire Anyone in the building more at risk, people Don’t block the projector light as can become hot
in the foundation studio- low risk
Electricity- tripping People in the same workspace – low risk Make people aware of cables keep cables out of the way tie them up
over wires
Electricity Electrical shock – low risk Un plug when not using

Important: During Production

1. Inform all persons exposed to risk of the control measures adopted (give a copy the risk assessment to all personnel /
authorities involved)

2. Monitor the activity and ensure that your control measures are in place and working effectively at all times

3. If conditions or your plans change, carry out a review of your Risk Assessment and apply new control measures if
appropriate. Record your changes.

4. Report any accident or near miss incident to the Media Centre and Academic Staff within 24 hours.
