Dear Mr./Ms. ___________________:

This is to inform you about the concerns raised regarding your performance and conduct as
_____________ It is necessary to address these matters promptly to ensure a safe and
professional working environment, and maintain the trust and confidence that is expected
from you.

In this wise, it has come to our attention that on November 12, 2023, the Management noticed
serious discrepancies in the daily sales tracker sheet for November 10 and 11, 2023. While
corrections were later on made, it was further discovered that there also errors in sales
recorded for November 7, 8 and 9, 2023.

In this wise, we direct that you provide a WRITTEN EXPLANATION regarding the above-
mentioned incident within ONE (1) DAY from receipt of this Notice. Your written response
should include an exhaustive and complete explanation of the circumstances surrounding the
issue, as well as all factors that we should consider.

Please be informed that your written response is for the purpose of getting your version of
the incident, and will be taken into account in determining any further actions or measures
that may be necessary.

Thank you for your attention to this matter. Should there be any questions or clarifications,
please do not hesitate to raise them to the undersigned.
