Sub: Request for help in issuing urgent visa for medical treatment at Medanta‐ the Medicity, India

Dear Sir,

Medanta‐ the Medicity is one of India`s largest multi‐super specialty institutes located in Gurgaon, a bustling town in
the National Capital Region. Founded by eminent cardiac surgeon, Dr. Naresh Trehan, the institution has been
envisioned with the aim of bringing to India the highest standards of medical care along with clinical research,
education and training. Medanta is governed under the guiding principles of providing medical services to patients
with care, compassion, commitment.

We have received a request from Patient Name: THIERRY WA KANUMUBADI KANUMBADI (Passport No.OP1200204) who
wants to come for detailed clinical evaluations followed by subsequent treatment under the care of Medical Oncology
Team.

Patient will be accompanied by attendant name: BRITHANY JOYCE TSIMBA KANUMUBADI (Passport No.OP1388388),
GISELE TSIMBA PHUKUTA (Passport No.OP0859630) and DORCAS PHEMBA PHUKUTA (Passport No. OP1045653) during
the treatment in India.

May we request you to kindly issue the visa on priority for the above mentioned patient and attendants after doing
necessary checks at your end as per your visa policy.
