drinking water Contamination. ln this i'egard, the foltowing instructions are
strict imptementation with immed'iate effect for prevention and control of seascnai'
diseases.

1. The CHO/MLFjp, MpHEO of FHCs are the mainl"y responsibte persons 1o utrlevt-'i':;
the controt and prevention of water Borne diseases. They shoutd administer 'i- -
checktist regarding the status of water sources in the Panchayaths of the Pl-"r['
scruputousty and action has to be taken as per contents given in tlre Checr'''i1:''r-
where ever necessary.

?. A Consotidateci Weekl.y Report shoutd be submitted by the CHO, MPHEC on the


Water Borne Diseases once in a week for the months March to May"
