Please note that delivery address MUST be in the Philippines ONLY. NO deliveries are made outside of the Philippines
Applicants who are being sent documents by the Embassy or who are dropping documents for the Embassy will have an
option to pick-up/drop-off documents for free or premium service. Applicants who are being sent documents will receive an
email or text message once the document is ready to be picked-up or delivered. Failure to provide accurate contact
information (phone numbers, address, and email) will result in the delay in the delivery of your document to the selected
location. Applicants have the following options to receive/send documents from/to the U.S. Embassy:

Option 1 – Free Pick Up/Drop-off: You may pickup/drop-off your documents free of charge at the following location:

Option 2 - Paid Pickup/Drop-off: You may select one of the 99 branches of LBC to pickup/drop-off your documents. Cost
of Premium drop-off/pickup is PhP440.00. Please see the list at https://www.lbcexpress.com/LBCBranchesODZ.

IF YOU SELECT PREMIUM DELIVERY:

All returned documents will be sent to the address entered below. Someone must be present when the courier comes to deliver
your passport.

- You must provide the exact address where the courier can deliver the documents and passport in "Mailing Address Line
1:".
- You must provide a local personal phone number in your contact details.
- Failure to provide these information will cause lengthy delays in the delivery of your passport.

Cost of Premium Delivery is PhP440.00 per passport. It is collected by the courier on delivery (Cash Only).
Please always provide exact change as the courier would only accept the exact amount.

 This letter (Interview Waiver Confirmation letter);
 Valid passport;
 Old passport with previous visa of the same class;
 DS-160 confirmation page;
 Two (2) photos (2x2-inch on glossy paper), white-background, unretouched, taken within
the last 6 months, in neutral facial expression, both ears and eyebrows must be visible.
DO NOT SUBMIT THE SAME PHOTOGRAPH THAT WAS USED ON YOUR PREVIOUS VISA.
Failure to submit a valid photograph will delay the processing of your visa application;

Additional Documents:
For first time B1/B2 applicants, below 14 years old:
 Original copy of birth certificate issued by Philippine Statistics Authority (PSA), and
 Photocopy of valid B1/B2 U.S. visa of either or both parents.
*SUBMISSION OF PARENT'S PASSPORT WILL NOT BE ACCEPTED.

For F or M visa applications:

 Approved Form I-20 from your U.S. educational institution
For academic J visa (students, professors, research scholars, short-term scholars, or specialists)
applications:
 Approved DS-2019 from your U.S. program sponsor
For Petition-Based visa (H, L, O, P, or Q) applications:
 Approved Notice of Action (/Form I-797)
For Crew visa C1-D / B1 Others applications (MAAP):
 Coast Guard letter for B1/OCS
 Manning Agency Guarantee Letter

ADDITIONAL INFORMATION:
Please note: Using the interview Waiver service does not guarantee visa issuance. In some cases, you
may be required to appear for a visa interview at the U.S. Embassy. If you submit an incomplete
and/or inaccurate application, or if you do not meet the criteria listed above, we will return your
application via the courier and you will be asked to schedule an interview. You should submit your
application well in advance of your planned travel, to allow for the possibility that a visa interview will
be necessary.
