AFFIDAVIT

I, Mohd Sadeeq S/o Bagh Ali R/o Bamyal Tehsil nagrota Distt
Jammu do hereby state an oath and declare as under-

1. That the deponent in the year 2013 to Mrs Rakhi Bibi and is
residing with her as resident son in law for the last over 9
years.

2. That the ration card of the deponent bearing ID No.

RCID11002444943 is no longer in use as the deponent resides
with his wife as resident son in law.

3. That the deponent wants to get entered into the ration card
bearing Id No. RCID011002431237 having members namely
Bharkat Bibi (mother in law of the deponent) and Rakhi Bibi
(wife of the deponent).

4. That the statements made above in para 1 to 3 is true and

correct and no part of it is false.

5. That this affidavit is solicited to the competent authority in the

concerned department for reference and record.
