(E) PROFESSIONAL REFERENCE: Are you still working? If not, when was your last job ended? Did you get
your final COE? (Please send the following information below:
a. Final COE (Certificate of employment)
b. For any candidates who don’t have their final COE, alternatives are okay
to send, what are the interim reqs for not having a COE? (resign letter
with MANAGER AND SUPERVISOR signatures, copy of exit clearance,
written documentation from the recent company for the
acknowledgement resign, signed and dated)

requirement will merit for a termination of employment.
