We hereby approved of this application and certify to its correction. In consideration of the benefits
to be derived from this activity, we expressly waive all claims against of any incidents, injury or
damage to personal/property that may occur beyond instituted in connection with the participation
of my son/daughter in the Boy Scouts of the Philippines – Dagupan City Council’s Scout Youth Forum
2024 which will be held on January 26-28, 2024 at Dagupan City Council Office.
I have reviewed the details of the forum, and I am aware of the purpose and agenda of the
event. I trust that the council will take all necessary measures to ensure the safety, well-being,
and positive experience of all participating scouts.
