AFFIDAVIT OF LOSS OF CERTIFICATE OF ACCREDITATION

I, EMMANUEL SALMOS DELA CRUZ, Filipino, of legal age, married, and a resident of North Pob
Nampicuan, Nueva Ecija, Philippines after having sworn to in accordance with law depose and state;

1. That I am the incumbent Chairman of Nampicuan Organic Producers Cooperative (NOPCO);

2. That NOPCO was issued a Certificate of Accreditation by the Department of Agriculture last 25
October 2019;

3. That the said Certification was in the custody of our previous chairman, Rogelio Zamora;

4. That chairman Zamora was removed from his position last June 2021;

5. That we inquired the whereabouts of the said certificate from him and even searched from our files
but all was for naught;

6. That I am executing this affidavit to prove that the Certificate of Accreditation issued in favour of
Sampicuan Organic Producers cooperative (NOPCO) is lost and is now beyond recovery.

IN WITNESS WHEREOF, I have hereunto set my hand this 20 th day of December 2022 in Palayan
