I, RENATO AGUSTIN y TALOSIG, of legal age, married and a resident of San

Fermin, Cauayan City (Isabela), after having been duly sworn o in accordance with law, deposes
and says:

1. That I am the registered owner of FNDY Blvd. Furniture & Sash Factory, situated at
Cauayan City (Isabela) with Certificate of Business Name Registration, Certificate No.
03364542 issued by the Department of Trade and Industry;

2. That the said business establishment is earning a gross income of One Hundred Thousand
Pesos (P100,000.00) annually;

3. That I am executing this affidavit to attest to the fact that said business establishment is
earning an annual gross income of One Hundred Thousand Pesos (P100,000.00) and for
any other purposes this statement may serve
