hereby solemnly affirm and state as follows:
That I am the deponent herein. My Son namely RAMPALLI PARTHUKUMAR RAJU, was born through
my wife namely UPENDRAM RENUKA, on 04-08-2007 at D.No.2/135A, Main Road, Amadagur
Village, Amadagur Mandal, Sri Sathya sai District, A.P., do hereby solemnly affirm and state as
follows:
By mistake and oversight, my Son date of birth was not egistered with the births and deaths
registration authorities of Tahsildhar, Amadagur Mandal. The said date of birth of my Son was enter
in his educational records and the same is genuine one. Except the said educational record there is no
other documentary evidence to show my Son date of birth. Hence Iam making this declaration for
grant of Date of Birth Certificate in favour of my Son by registering the same.
3) What all mentioned above is true and correct to th¹ best of my knowledge and belief. I have
not misrepresented any fact in this affidavit.
