AMENDED BY THE MAJORITY VOTE OF ALL MEMBERS


OF THE BOARD AND ASSOCIATION REGARDLESS OF
STANDING

 Application letter to amend.


 Articles of Incorporation and/or Bylaws as amended, underscoring or underlining changes
made;
 Notarized Board Resolution approving and adopting the proposed amendments, with the
amendments clearly stated therein, that the same were approved by majority vote of all the
Board members at a meeting where there was a quorum, and the date of the said meeting;
 Notarized Secretary’s Certificate attesting that the amendments were approved by the majority
vote of all members of the association regardless of standing, and stating therein the date of
the members’ meeting and the fact of quorum;
 Minutes of the meeting of the Board called for the purpose duly certified as true and correct by
the association’s secretary;
 Minutes of the general assembly called for the purpose, duly certified as true and correct by
the association’s secretary;
 Attendance sheet of the general assembly signed by the members present and indicating
therein their corresponding votes.
 Authorization for the representative of the association to transact and/or follow-up its
application with DHSUD;
 Proof of payment of filing fees.

INCOMPLETE REQUIREMENTS WILL BE RETURNED TO APPLICANT/WILL NOT BE PROCESSED.
3. Mark “/” for submitted documents and “X” for lacking documents.
4. DHSUD-HOACDD reserves the right to request additional requirements to facilitate the
registration evaluation process.

STANDING

 Application letter to amend.


 Articles of Incorporation and/or Bylaws as amended, underscoring or underlining changes
made;
 Petition to amend duly signed by the thirty (30%) of the members in good standing submitted
and received by the ELECOM;
 Notice of meeting indicating the date, time and venue of the special meeting;
 Proof of posting of the notice of meeting that it was posted at the bulletin board of the
association and conspicuous places within the subdivision/village or community;
 Notarized certificate of Election Committee attesting that the amendments were approved by
the majority vote of all members of the association regardless of standing, and stating therein
the date of the members’ meeting and the fact of quorum and approval of the proposed
amendments;
 Minutes of the special general assembly meeting stating the purpose of deliberation and means
of approval or disapproval of the proposed amendments;
 Attendance sheet of the special general assembly meeting signed by the members present and
indicating therein their corresponding votes.
 Proof of payment of filing fees.
