AFFIDAVIT OF UNDERTAKING

Affiant, ___________________ , a Filipino, of legal age, with postal

address at (Address), after having been sworn in accordance with law hereby
deposes and states that:

1. Affiant is the HR Manager of (Name of Company). (the

“corporation”), a corporation duly organized and existing under and by virtue
of the laws of the Republic of the Philippines, with operating business address
at (Company Address).

2. One of the requirements in securing of an Alien Employment

Permit is the Mayor’s Permit. The Corporation is currently working on the said
document with the Makati City Hall and the status is still on process.

3. Affiant hereby undertakes to provide copies of the Mayor’s

Permit under the name of the Corporation issued by the Makati City Hall
within three (3) months or until 30 April 2023.

4. Affiant is executing this affidavit to attest to the truth of the

foregoing and for whatever legal purposes it may serve.
