2. Pendaftar akan menasihati anda mengenai kelulusan nama. Setelah diluluskan, Sijil Kebenaran Penggunaan Nama

The Registrar will advise you of the approval of the name. Once approved, Certificate of Name Approved will be issued and will be
send by email to the applicant or to the Company. The ship’s name will be valid for registration for six (6) months from the date of

3. Suffix berdaftar adalah nama awalan sesebuah kapal yang telah didaftarkan, prefix berdaftar adalah nama akhiran
sesebuah kapal yang telah didaftarkan. Nama prefix atau suffix hanya boleh digunakan oleh pemilik yang berdaftar
sahaja
Registered prefix is a registered name for the first name of the ship and registered suffix is a registered name for the last name of the
ship. Registered prefix or suffix shall not be used by the other person (s) / company (s) it can be used by registered owner (s) only.

Lantikan atau pembatalan Pegawai Yang Dibenar bagi sesebuah syarikat perlu dibuat oleh Setiausaha
Syarikat berkenaan. Meterai syarikat hendaklah dilekatkan kepada Borang ini sebagai persetujuan.
Appointment or cancellation of an Authorised Officer for a Company must be made by the Company's Secretary. The Seal of the
Company must be affixed to this form as a sign of agreement

2. Maklumat Pegawai Yang Disahkan (Informations of Authorised Officer)

Nyatakan nama dan alamat penuh Pegawai Yang Disahkan. Alamat yang digunakan hendaklah alamat
rumah atau pejabat berdaftar. Jangan gunakan alamat yang menggunakan Peti Surat

1. Lantikan Pengurus (Appointment of Manager)

(i) Hanya satu Pengurus sahaja yang boleh dilantik bagi sebuah kapal didalam satu satu masa.
Only one Manager may be appointed for one particular ship at any one time.

(ii) Pemilik boleh melantik individu atau Badan Korporat sebagai pengurus yang sah bagi
kapal tersebut. Pengurus juga boleh disama ertikan dengan takrif yang dinyatakan didalam
Kod Pengurusan Keselamatan Antarabangsa.
Owner may appoint individual or Body Corporate as Manager for the ship. Manager may have the same definition as
that specified under the International Safety Management Code.

2. Pengisian Borang (Filling of Form)

(i) Pemilik Individu perlu mendaftarkan lantikan dan pindaan Pengurus secara terus dan bukan
melalui pihak ketiga dengan mengisi dan menandatangani Borang ini. Pegawai Yang Dibenar
perlu berbuat demikian bagi pihak Pemilik Badan Korporat.
Individual Owner must register appointment and amendments of Manager directly and not through third party by
filling and signing this form. Authorised Officer must do so on behalf of a Body Corporate Owner.

(ii) Jika borang ini digunakan untuk memaklumkan mengenai pertukaran Pengurus, butiran
Pengurus baru hendaklah dinyatakan didalam ruangan Butiran Pengurus. Pengurus lama akan
menjadi terbatal secara otomatik
If this form is used to notify change of Manager, the particulars of the new Manager should be indicated in the
Particulars of Manager's column. Previous Manager shall automatically be deleted.

3. Alamat Pengurus (Address of Manager)

Alamat Pengurus hendaklah alamat tetap pengurus berkenaan dan hendaklah alamat berdaftar syarikat

1. Am (General)

Deklarasi ini membolehkan Pendaftar Kapal Kapal untuk menetapkan hak milik keatas Kapal yang
ingin didaftar. Deklarasi ini mesti ditandatangani oleh setiap Pemilik.
Jika deklarasi ini dibuat oleh Pegawai Yang Dibenar, borang DKapal 4/98 yang lengkap perlu
disertakan sekali This Declaration enables the Registrar of Ships to establish the entitlement of a ship to be registered.
This Declaration must be signed by every owner.
If this Declaration is to be signed by the Authorised Officer, Form DKapal 4/98 duly filled must be submitted

6.1 In a visible place on the stern / side of the hull, amidships port and starboard, above the
deepest assigned load line / side of the superstructure, port and starboard / on the front of
the superstructure / in the case of passenger ships, on a horizontal surface visible from the
air.

6.2 In the easily accessible place on one of the end transverse bulkheads of the machinery
spaces / on one of the hatchways / in case of tankers, in the pump room / in the case of
ships with ro‐ro spaces, on one of the end transverse bulkheads of the ro‐ro spaces.
