 The PSSR is a safety review conducted prior to startup
(Commissioning) of a new or modified Processing / Manufacturing
plant or facility or after shutdown activities to ensure that installations
meet the Original Design or Operating Intent, to catch and re-
assess any Potential Hazard due to changes during the Detailed
Engineering and Construction phase of a project.

 In other words, it ensures the “Ready for Start-up or Operation


Readiness ” status of Process Facility/Units.
 A PSSR covers not only Equipment, but also Procedures and
Training.

 Conducting “Pre-start up safety review “PSSR is an essential


element in achieving operational readiness.

 PSSR procedure/guideline is normally prepared as part of Pre-


commissioning / Commissioning documentation.
 To ensure that installation meets the original design of process plant/facility.

 To ensure safety, operating, maintenance and emergency procedures are in


place and adequate.

 To ensure appropriate safety reviews during engineering phase (HAZOP


study, model review etc.) have been carried out and all action items have
been completed.

 To ensure any site modifications during construction phase have been


properly controlled and noted.
 To ensure training of each employee involved in the operating process is completed.

 In addition, the PSSR plays an important role in large projects as it may give plant
operators the authority to refuse or accept responsibility for a plant or equipment
that they judge to be unsafe through the PSSR.

 Ensure construction and equipment is in accordance with design


specifications and a Punch List of safety and administrative items which
need to be Completed before startup is Prepared.

 For changes involving the introduction of a Highly Hazardous Chemical,


ensure that the above items have been completed prior to the introduction
of highly hazardous chemicals.
1. Coordinate PSSR participation from knowledgeable Contractor and
Owner personnel.
2. Conduct Technical and HSE reviews of documentation and on site.
3. Verify all construction and equipment is in accordance with safety,
environmental and health regulation and standards.
4. Complete all corrective action items raised in the PSSR.
5. Complete Post- Start-up corrective action items.
6. Representative of the Project Owner sign the PSSR checklists for
approval of commissioning/start-up of the plant.
7. Documentation of PSSR.
 The PSSR process consists of :

1. PSSR requirements.
2. Assembly of PSSR team.
3. PSSR execution.
4. Tracking of corrective actions.
5. Authorization of PSSR.
6. Documentation of PSSR.
The intent of the PSSR process is to ensure that it is safe to operate equipment. So,
the process must ensure:
 Construction of equipment is in accordance with design specifications.
 Safety, operating, environmental, maintenance, and emergency procedures
are developed, adequate and authorized.
 Process Hazard Analysis (PHA) and/or HAZOP recommendations are resolved.
 Training completed.
 Process safety information has been updated.
 General requirements for safety, health, and environmental considerations
are adequate (Signs, safety showers, FF equipment, Emergency equipment)
 The asset owner is aware of the review and approves the facility as safe to
operate.

Maintenance
 The PSSR leader will need to ensure that the documentation for
the changes is readily available for the team.

 The PSSR leader should lead the team in completing the PSSR
checklist.

 For each portion of the checklist, the team must determine if the
status of the item is adequate to allow the unit to start safely.

 If the team determines that there are substandard conditions,


they must identify the corrective action and make action plan
and follow it.

 Equipment cannot be started until all corrective actions that


require resolution prior to start up are verified as completed.
 All corrective actions that are identified as part of the PSSR
process must be tracked using Action Management.

 Priority on resolution of these corrective actions should be


based on the residual risk associated with the items.
 Once the PSSR is complete and all corrective actions required
for safe startup of the equipment is complete, the PSSR leader
will seek out the necessary approvals of the PSSR.

 There are a number of approvals required prior to


commissioning of new or modified assets.

 The following individuals must complete the approvals and


authorization section of the PSSR checklist:
- PSSR leader. - Project manager.
- Area environmental advisor. - Technical specialist.
- Operations maintenance coordinator.
- Asset owner / operations manager.
 The final PSSR document must be stored with the
project files for the asset.

 After all of the outstanding action items relating to the


PSSR have been completed, the project manager is
responsible for scanning the document into the
project information file.

 The files should be stored in the “Environmental Health


& Safety” section of the project folder.
