We are pleased to offer you the position of XXX. You will be based in XXXX, Terengganu and
your commencement date shall be on the xxx. Listed below are the terms and conditions
governing your service. If you find them acceptable, please sign and return the duplicate copy of
this letter to us within xxx (xx) days from the date of this letter.

MEMORANDUM OF TERMS AND CONDITIONS OF EMPLOYMENT

Position : xxx

Appointment Term : Contract

Salary : RMX,XXX per month

Probationary Period : A probationary period of one (1) month will apply from the
date you commence employment with the Company. During
the probationary period, either party may terminate this
contract of service with one-week notice period or payment of
salary in lieu of notice.

Allowance : Site allowance of RMXXX per month.

The Company will provide you with a mobile phone package


as a phone allowance.
EPF & SOCSO : You will be eligible for Employee Provident Fund (“EPF”)
contribution at the present rate of X% of your monthly salary
and the Company will contribute X% (Statutory Rate).
SOCSO will be deducted as per salary schedule.

Bonus and Increment : The Company at its discretion will provide bonus and
increment to employees based on individual performance of
the employees as well as overall performance of the
Company.

Medical Benefits : The Company will meet the cost of medical treatment for you
and your family (one legal spouse and unemployed children
under the age of 18 years old) with outpatient medical
consultation and treatment subjected to a maximum of
RMXXX/- per annum. This includes medicine and
hospitalization as prescribed by practicing doctors recognized
by the Company. The Company will also bear dental
expenses for yourself of up to RMXXX per annum.

Public Holidays : Gazetted public holidays applicable to [state] and Malaysia as


a whole.

Annual Leave : Eighteen (18) working days.

Termination Notice : Three (3)-month notice period or salary in lieu of such notice
Period for both parties.

Performance : You must show satisfactory performance during your


employment period. Your supervisor will conduct yearly
performance assessment and agree on KPIs for the following
year.

Code of Conduct : All employees shall observe and adhere to the following:

Faithful and diligent at all times in performing duties and


responsibilities as may be assigned to you by the Company,
and at all times promote and advance the interest of the
Company.

Obey and comply with all orders and directions given to you
by the Company, and faithfully observe all rules, regulations,
procedures, practices and policies of the Company, whether
expressed or implied, for the time being in force for the
management of the Company’s business in all respects.
Not any time to be guilty of any act or conduct causing or
calculated to cause damage to the Company, its property,
reputation of general interests and in all respects and at all
times conduct your yourself with propriety and decorum.

Not at any time during the continuance of or after the


termination of your employment with the Company except
with the express approval of the Company, divulge either
directly or indirectly to any person or company, knowledge or
information which you may acquire during the course of or
incidental to your employment by the Company concerning
the affairs or property of the Company or its associated or
subsidiary companies or any business or property or
transactions or policies in which the Company or any
associated or related company may be or may have been
concerned or interested.

Summary Dismissal : The Company reserves the right to dismiss an employee for
serious misconduct, including drug abuse and dependency,
dishonesty, insubordination, gross neglect of duty, criminal
conviction, bankruptcy or any other default, without notice,
salary or other forms of compensation.

The Company also reserves the right to terminate an


employee either during probationary period or after
confirmation, if it is discovered that the said employee had
made a false or fraudulent declaration, representation or
statement in his or her application form, resume or during
interview for the particular position.

Management within seven (7) days from the date of this letter, failing which this offer will
automatically lapse.
