An application is hereby made in the High Court of the Supreme Court of Judicature
of Guyana on behalf of MARGE PHILLIP, Chief Executive Officer, widow, of 164 Fern Gully,
Georgetown, Demerara for a grant of probate of the Will dated March 9th, 2012 of the above
named deceased who died on testate on September 8th, 2012, the said deceased having appointed
MARGE PHILLIP as the sole executrix under the said Will.

OATH OF EXECUTRIX
I, MARGE PHILLIP, Chief Executive Officer, widow, of 164 Fern Gully, Georgetown,
Demerara being duly sworn make oath and say as follows: -

1. That I believe the paper writing dated March 9th, 2012, marked and deposited by
me in the Registry of the Supreme Court on the 14th day of November 2017, to contain
the true and original last Will and Testament of MARK PHILLIP, deceased, late of 164
Fern Gully, Georgetown, Demerara, who died testate on the 8th day of September, 2012 at
the Georgetown Public Hospital, Georgetown, Demerara, he being domiciled in Guyana
at the time of his death having a fixed place of abode at 164 Fern Gully, Georgetown,
Demerara.
2. That I am the sole Executrix of the said Will of the said deceased and I will
administer according to law all the estate of the said deceased which comes into my
hands to be administered and I will render a just and true account of all my dealings
therewith whenever called upon so to do and that the said estate amounts in gross value to
the sum of $40,500,000 and no more to the best of my knowledge, information and
belief.
3. That hereunto annexed and marked “A” is a true statement of the Assets and
Liabilities of the said estate and I have exhibited in the Estate Duty Section of the Inland
Revenue Department a true and correct inventory of the said estate for the purpose of the
payment of process fees thereon.

AFFIDAVIT OF WITNESS TO EXECUTION OF WILL

I KARL HATAKE, Personal Trainer, of Lot 77 Bacchanal Drive, Georgetown, Guyana being
duly sworn make oath and say as follows:-

1. That I am the Deponent herein.

2. I was present on 9th day of March, 2012 together with DEO UZAMAKI of Lot 89
Hidden Leaf Street, Georgetown, Guyana, both of us being over eighteen (18) years of
age, at No. 77 Electric Avenue, Georgetown, Guyana, did then and there see the said
MARK PHILLIP sign and publish his last Will and Testament which said Will is
annexed to the affidavit of the Executrix. A copy of the said Will is now produced and
shown to me and hereto annexed and marked “K.H.1”.

3. I and the said DEO UZAMAKI then and there together sig ned and attested the said Will
as witnesses to the signing and publishing thereof by the said MARK PHILLIP in his
presence and in the presence of each other.

4. The signature “M. Phillips” at the foot of the said Will is of the true handwriting of the
said MARK PHILLIP, deceased.

5. The signatures of “Karl Hatake” and “Deo Uzamaki” to the said will subscribed as
those of the witnesses attesting the signing and publishing thereof by the said MARK
PHILLIP are the true handwriting of me this Deponent and the said DEO UZAMAKI
respectively.
