To Whom It May Concern,

This is to certify that according to his/her records in this hospital, BERT A. SISON , 48 yrs. Old,
of Lower Engineer’s Hill has been attended to by Dr./Drs. Ton Dizon on January 5, 2024 at 6:05
pm.
Findings:
1.) Upon admission to the Emergency room, patient shows no signs of life. No vital signs
were determined.
2.) Patient shows multiple contusions at the anterior and posterior chest and abdomen and
visible hematoma on the right parietal area of the head. Visible hematomas are spotted
on the right side of his face with contusions appearing on the upper right area of the eye
and left side of the neck and jaw.
3.) Patient appears to have suffered injury and internal bleeding in the right parietal area of
the head.
4.) Patient has no water in the lungs and skin does not show wrinkling due to water
submersion.
5.) Cause of Death: Internal bleeding and severe brain injury secondary to blunt head
trauma secondary to mauling.
6.) Not treatable.
It is hereby concluded that Mr. BERT A. SISON who was brought to the emergency room is
hereby declared CLINICALLY DEAD ON ARRIVAL at 6:05 in the evening.
