TERMS OF REFERENCE

CONTRACT OF SERVICES – SERVICES OF COVID-19 (RT-PCR) TESTING FOR DLSU EMPLOYEES


& ESPs’
(COVID-19 TESTING PROGRAM)

1. Scope of Work

The contractor shall conduct the Covid-19 testing to all current and incoming members of the
Lasallian community including but not limited to the following: (See appendix A)
2.1. Administrators
2.2. Faculty
2.3. Academic Service Faculty (ASF)
2.4. Administrative and Professional Service Personnel (APSP)
2.5. Co-Academic Personnel (CAP)
2.6. Direct-hire personnel
2.7. Agency –based personnel
2.8. External Service Personnel (ESP)

Pursuant to Department of Health (DOH) Department Memorandum No. 2020-0220, Re: Interim
Guidelines on the Return-to-Work. The University intends to conduct selective Reverse
Transcription-Polymerase Chain Reaction (RT-PCR) and Antigen tests on its employees and, to
this end, contract services of a service provider who will assess, diagnose, recommend and,
where applicable, subject DLSU employees to RT-PCR and Antigen test for detection of COVID-
19 Virus.

2. Contract Period

The contract shall be valid for a period of three (3) weeks (three (3) sessions per week / 300
employees per day) from 6:00 am to 6:00 pm, Monday to Friday. The test can go beyond 6:00pm
until 8:00pm to serve the night shift employees. In case of “closed campus” announcement, there
is a provision of rescheduling the test to the nearest date available.

3. General Requirements

A. DLSU shall require the test subject to applicable DOH guidelines, refer suspected employees to
its DOH-licensed partner laboratory for RT-PCR and Anti-gen testing and facilitate testing of
DLSU employees who are considered “frontliners” and are at high risk.
B. Ensuring the credibility and accuracy of the Covid-19 testing is critical to the University, thus only
a DOH-accredited drug test laboratory shall be utilized.
C. Diagnostic test laboratory shall furnish a copy of their certification of Accreditation from DOH to
conduct the drug testing.
D. The drug testing shall be conducted in strict conformance with the procedures prescribed by the
Department of Health (DOH).
E. DLSU shall provide the venue for the collection site in conformance with the DOH guidelines and
approved by the accredited drug test laboratory
F. Strict confidentiality shall be observed with regard to the Covid-19 test process and result.
G. The date, venue and list of persons to undergo the Covid-19 test will be determined by the Covid-
19 Testing Coordinator
H. There is no numbering system provided. First come, First serve basis is observed from those
group or individuals on the list to be tested on that scheduled day.
I. Priority lane is provided for the VIPs, PWD and Senior Citizen members of the Lasallian
community.
J. The official result of the Covid-19 test is submitted directly to Covid-19 Testing Coordinator.
K. The Covid-19 test results will be released immediately after three (30 days upon testing was
conducted for the RT-PCR and after 48 hours for the Antigen test . It shall be stored in a
database that can only be accessed by the Covid-19 Test Coordinator to protect against unlawful
access, fraudulent misuse, unlawful destruction and alteration.
L. For negative Covid-19 test results, the retention period of the original copy of result shall be for
one (1) year in conformance with the validity of the test result.
M. For positive Covid-19 positive results, the retention period of the original copy shall be as long as
necessary for the fulfillment of the purpose for which the result was obtained, and in aid of the
procedure and medical intervention prescribed.
N. Covid-19 Testing Coordinator created a comprehensive Health Contact Tracing Declaration Form
to be distributed and filled out by all DLSU employees and ESPs
O. The disposal of the Covid-19 test results shall be the responsibility of the Covid-19 Test
Coordinator. Data shall be disposed in a secure manner to prevent unauthorized access or
disclosure to ensure that appropriate protection to personal data is provided.

Scope of Work:
1. Existing program on Covid-19 - Referral of DLSU Employees
 Assist the OPM in drafting a comprehensive Health & Travel and Contact Tracing Declaration
Form to be distributed and filled out by all DLSU employees
 HSO team comprise of licensed medical doctor who will assess the Forms submitted by all DLSU
employees
 Consultation with the medical doctor will be conducted by the HSO team through telemedicine or
in person (if requested by the employee and is necessary under the circumstances) on CoViD-19 related
matters.
 Doctor’s Recommendation for patients who will need to undergo an RT-PCR/Swab and Antigen
Test.
 Subject to the criteria, referral to DOH accredited testing facilities who will conduct RTPCR and
Antigen Testing for DLSU employees recommended to undergo Antigen and RT-PCR
Testing.
 Facilitation of RT-PCR and Antigen Testing on DLSU employees who are high risk (Frontliners)
 Online accommodation for absentee employees, referral to RT-PCR/Swab and Antigen Testing.
 Periodic evaluation of DLSU employees, those who were recommended to be tested for RT-PCR
Test and those who might have developed symptoms or who had contact with a suspected positive
COVID patient or positive COVID patient.
2. RT-PCR Laboratory Test
 Collect swab samples from employee
 Conduct RT-PCR Test due to work requirements
 Release result within 2-3 days from collection.
3. Antigen Laboratory Test
 Collect samples from employee
 Release result within 24 - 48 hours from collection.

STEPs in collection of specimen:


A. For saliva RT-PCR test
1. No mouth activity for 30 minutes prior saliva collection
 No brushing, drinking, eating, smoking, vaping, putting lipstick, etc.
 Specimen should be clear & non viscous. No discoloration, blood, food debris, phlegm or
any impurities.
 Fill at least half of the tube (1ml). Otherwise result may be INVALID.
2. Upper respiratory tract
 Nasopharyngeal specimen (NP) collection /Oropharyngeal (OP) (throat) specimen collection
(performed by a trained healthcare provider, only)
