AFFIDAVIT OF LOSS

I, RICHARD Q. ALVISO, of legal age, married, Filipino, with office address at Cedaville
Residence, Silver Hills Subd. Talamban, Cebu City after having been duly sworn to in accordance
with law hereby depose and state that:

1. I am a holder of National Transmission Corporation Identification Card with

number 15008-E
2. Sometime on March 06, 2024, at La Carlota, Negros Occidental, I asked a Certificate
of Appearance in Court, and I gave my ID. When it was returned to me, I did not
notice where I placed it. When I arrived in Cebu, I found out that the said
Identification Card was missing.
3. My diligent effort to locate the said Identification Card had proved futile and
unavailing and I had declared that the said Identification Card is forever lost.
4. I further attest that the said Identification Card was neither surrendered nor
pledged to anybody.
5. I am executing this affidavit to attest to the truth of the foregoing facts, to
support my request for the issuance of a new Identification Card with the
National Transmission Corporation.
