REASON FOR REFERAL/PURPOSE

The client is presently working as a Special Education Teacher at

Paaralang Sentral ng Mataasnakahoy. She is married with one son. Based on
client’s interview, she has a good socio-emotional and intra personal
functioning.

The purpose of this test is for psychological assessment of the client

to determine her present emotional status.

BEHAVIORAL OBSERVATION DURING EXAMINATION

PHYSICAL APPEARANCE

The client is a 32 y/o female. She has a fair- round thin face with some
dark spots in her cheeks. She has wide forehead and black round eyes with
small eyebrows. Her nose is small and straight. Her hair is long, brown and
wavy with many double ends. She is thin and her body, legs and arms are
small. Her nails are short with a white tip, showing how she bite them
frequently.

The client is wearing a white shirt, ragged pants and a white sneakers.
She looked very comfortable in her outfit. She seemed happy and at ease as
she smiles to me when she greet.

AFFECT

During test administration, the client’s socio emotional and

interpersonal functioning is pleasing based on initial interview made.

JUDGEMENT

The client’s activity level during test administration is normal. No

unusual mannerism were noted and her eye contacts are within normal limits.
Her judgement were observed to be normal and congruent. No overt
indications of negative behaviour were observed during the examination.

TEST RESULTS

Attitude towards Father

The client sees her father happy with what his children are achieving
right now. Her father cannot stand for his family due to lack of education.
Despite this, she wishes her father gain his good health.

Attitude towards Mother

She views her mother as a hard working woman. She admires her
mother even though they are not close to each other. She experienced
misunderstandings with her mother than with her father.

Attitude towards Family Unit

For the client, her family treats her as an important part of it. She
describes her family as simple but happy and has a god relationship and good
bonding moment with family members.

Attitude towards Women

The client views women as a powerful and can be a leader. The client is
strong, independent, can stand on her own and does not rely her life to man.

Attitude towards Heterosexual Relationship

She has a positive attitude towards heterosexual relationship. She views

married life as a commitment that requires patience and true love between
partners. The client’s sexual life is boring.

Attitude towards Friend and Acquaintances

The client is longing for a loyal and true friend. She is a true friend and
she doesn’t like to be used or taken for granted by her friends and colleagues.
Trustworthiness and faithfulness are important to her.

Attitudes towards Superior at Work

She has a high regard with her superior. To her, if you are a leader you
should guide your members to a right track. It frustrates her knowing her
superiors are not doing good or supporting them as part of the team.
Attitudes towards People Supervised

The client shows a desire of being a leader. She is goal oriented and has
a strong will to manage a group. She has a high respect for her group members
as she consider them as the people she loves.

Attitude towards Own Ability

The client views herself as someone who is faithful to God. In everything she
does, she prays, during good and bad times she prays. It seems that she loves
her son so much that she indicates that he is her weakness. She believes that
she has the ability to make change for the world.

Attitude towards the Past

She finds her childhood as happy with her best friend who
always share toys to her. Her memories with her best friend is the
highlight of her childhood. She has a regret of not studying so hard
when she was a child and indicated that if she will be young a gain,
she will not be shy in front of other people and will learn to express
her feelings.

Attitude towards the Future

The client sees the future as bright as the stars. She has a strong desire
to reach her dreams and dreaming a happy and the brightest life can offer so
when she grows old she will reminisce all her achievements and happy
moments happened to her.

Attitude towards Goals

The ultimate goal of the client is to become a supervisor. She is happy

when her family is happy too. She doesn’t want anything except to provide
everything and anything her family needs and wants.

SUMMARY OF FINDINGS

The client shows respect to her father but recognizes some lacked
abilities. She admires her mother but misunderstanding arise. She views her
family as simple but happy . She expressed high regards with women and
believes that women can stand on their own. According to her, a married life
is a commitment that requires patience and true love between partner. She
values loyal and true friends.

RECOMMENDATION

The following are recommendations based on the findings:

 The client may continue focus on the things that make her happy.
 The client may engage more bonding moments with her parents
specially her mother.
