HSE Slip and Trip Injury


ALERT While Handling Tagline

What happened?
While holding a tagline for handling a bundle of pipe, a
sudden tug caused the tail end of the load to turn. When
the load came back in opposite, the roustabout (IP) lost his
footing, slipped and fell to the deck, injuring his arm.

Consequences:
● Work-related injury

Why did it happen?


● Deck floor was slippery due to poor housekeeping
● Lack of hazard recognition

What should be done to prevent reoccurrence?


● Housekeeping shall be conducted before, during, and after performing any task.
● Hazard recognition refresher training to be delivered to all crews.

Areas where improvement is needed:


Workers must:
● Understand the work permit scope, conditions, and limitations.
Lifesaving Rule No. 1
● Immediately stop work if unsafe conditions arise and inform the issuer.
● Make sure the job site is returned to a safe condition and close the work
permit with the issuer.
Work supervisors must:
● Confirm that all hazards are identified and associated risks are controlled,
including those involving simultaneous operations.
● Confirm that permit conditions are met and all required resources are at
Work Permit the worksite before work starts.
● Frequently check the worksite to ensure compliance with work permit
conditions.
