DESCRIBE THE POTENTIAL INCIDENT/ HAZARD/ CONCERN AND POSSIBLE OUTCOME

Two construction workers, Mr. Almer Ramirez and Mr. Jonel Aves, were injured in a safety incident that happened at
the Future Heights Building Project construction site in Palayan City, NE, on April 03, 2024, at approximately 9:30 AM. The
incident involved a scaffold collapse. The scaffold unexpectedly collapsed, plunging the workers about ten feet to the ground
below. The scaffold was intended to support laborers and materials during the external finishing of the second level. The on-
site safety officer acted quickly, contacting emergency services and having qualified coworkers provide first aid to the injured
workers.
Emergency medical responders arrived, gave Mr. Ramirez and Mr. Aves more medical attention, and took them both
to Palayan City Hospital. While Mr. Aves suffered multiple bruises and a concussion, Mr. Ramirez had treatment for a fractured
wrist. It is anticipated that both will fully recover. Together with local safety inspectors, the site safety officer initiated a
preliminary inquiry into the occurrence. According to preliminary investigations, overloading and the use of faulty support
beams—which were overlooked during the regular safety inspection earlier that week—may have contributed to the scaffold
collapse.

SAFETY SUGGESTION:

Right after the incident, all construction was temporarily put on hold to enable a thorough safety audit of the site.
A comprehensive examination of all the scaffolding, tools, and equipment utilized on the job site was part of this. Additional
safety training will be provided to all construction workers, with an emphasis on emergency response protocols, hazard
identification, and scaffold safety. To avoid such mishaps in the future, corrective actions have been put in place, such as the
prompt replacement of any damaged equipment and the implementation of stricter safety inspections. Additionally, the
construction company has pledged to actively collaborate with safety inspectors in order to assess and improve current safety
procedures.
The incident serves as a somber reminder of the vital necessity for strict safety procedures in all phases of
construction activities. The safety and well-being of all personnel are of the utmost significance. This study is part of a
continuous effort to raise safety standards in the construction sector and will be submitted to the relevant regulatory
organizations.
