INCIDENT DETAILS

Description of machine, process or type of work involved:

A leakage from the sink flexible hose.

Description of incident:
• A code grey was announced from OT# 04 at 03:27 hours stating water leakage.
• In response to the code grey Engie team immediately went to the location mentioned and found
that the was leaking through the sink hose.

Action Taken

• To contain the leakage, the isolation valve under the sink was shut off immediately.
• A soft service team was requested to clean the floor surroundings to prevent any potential
hazards or inconvenience.
• The damaged sink flexible hose was replaced with a new one.
• After replacing the damaged hose, the isolation valve was opened to restore water pressure.
• Code Grey cleared at 04:06 PM.
