Short Description: 4’’ pipe slips

while manual handling

Work Activity: pipe painting for

chiller line

Event Description: On the above-mentioned date, time & location. HEXATECH mechanical team were tasked for
painting the pipes. The operatives taking 4’’ pipe from stacked area for painting. At that moment one worker take the
pipe from pipe stacked stand suddenly pipe twisted & falls from the height around 1 meter height, weight around
35kilogram, 4-meter length. During the sudden falling the worker get injured in his right shoulder

During the investigation of the incident knows that its pure negligence of co – worker fail to communicate while manual
handling the pipes

Immediate Action(s) Taken

 Involved person {IP} taken to HBK clinic for first aid & confirmed that shoulder bone is not broken
 Informed the concerned Engineer & supervisor
 Arranged vehicle to IP shifted to Hamad hospital for further medical

Corrective / Preventive Action(s) Plan

 Standdown meeting conducted with HEXATECH operatives along with Engineer & Supervisor
 While manual handling the any heavy objects the work in charge person must standby in that area
 Must need proper communication with the co- worker while manual handling
