What happened:
On 23rd June 2021, the injured person (IP) was operating the EOT crane with remote for shifting the gantry
portal. During shifting the gantry portal, it struck on his right leg resulting in an injury on his right leg. IP was
immediately taken to First Aid Centre and then referred to TMH for further treatment. In X Ray, the fracture
was revealed in his right leg.

Preliminary Findings:
1. Crane operator was working with remote operated EOT crane.
2. Housekeeping in the area was found adequate.
3. Illumination at the incident site was found adequate.
4. Job was being supervised.
Immediate actions taken:
1. The IP was immediately taken to the First Aid Centre and thereafter he was referred to TMH for
further treatment.
2. Incident was communicated to all concerned employees.
3. Incident was logged in Ensafe (PIR - IN00118205) system for detailed investigation.
Recommendations:
1. Six directional hazards analysis should be done before starting the job.
2. Correct slinging method should be adopted, keeping in view the weight and centre of gravity of
the load.
