What happened:
On 20th July’21, the injured person (IP), a dumper operator was in night shift duty. He was deployed for
transportation of friable ore from Katamati Iron Mine to Wet plant dumping yard at Noamundi. At around
12:30 AM, after completion of 3rd trip, he was returning to Katamati mine with empty dumper. On his way,
at RL 636 of Hill-6, he felt asleep and lost his control over the dumper. The dumper went over some boulder
materials stacked on the bench side. IP felt back pain due to the jerk caused by the incident. He was
immediately taken to Noamundi hospital where the doctor on duty gave him necessary medicines and
released him to join duty. He came back to the mine and completed his night shift duty. On 21st July’21, at
around 5.30 PM, he informed his superior of having back pain and visited Noamundi hospital for treatment.
Doctor on duty advised him for X-ray and declared unfit for 2 days.
Preliminary Findings:
1. The width of the haul road near the incident site is more than 25 meters.
2. Illumination at the incident site was found adequate.
3. The fatigue monitoring system of the equipment was working, however, it’s integration with FMS
was not done.
4. Alcohol test has been conducted and IP was found negative.
Recommendations:
1. Communication shall be made to all the employees for taking proper rest before coming to night
shift duty.
2. Fatigue monitoring system shall be integrated with Fleet Management System for generation of
real time alerts at control room.
