    A contract worker named Mr. Pradip Mandal suffered fatal burn injuries while inspecting a clogged strainer in the secondary combustion chamber of an incinerator plant. During the inspection, hot gas suddenly came out of the coke oven gas pipeline when they opened the strainer, burning Mr. Mandal. He was taken to the hospital but later succumbed to his injuries. A preliminary investigation found that standard operating procedures were available for the job and Mr. Mandal had experience operating the incinerator plant since 2016. The company is providing assistance to Mr. Mandal's surviving family members and has formed an investigation team to further examine the incident.

    Incident : Fatal incident of Mr. Pradip Mandal - a contract employee


    Date of Incident : 24.02.2023 Time: 01:45 PM
    Department : Mills Mechanical Maintenance, Shared Services
    Section : Utility, Cold Rolling Mill (CRM)
    Location : Platform of Secondary Combustion Chamber of Incinerator
    Details of Deceased Person : Mr. Pradip Mandal, Age: 35 years, Workman of M/s GAPSPL
    Injury : Burn injury on both hands and legs.

    What Happened:
    M/s GAPSPL is deployed for the operation and maintenance of Incinerator Plant at CRM. Mr. Pradip Mandal
    (Deceased Person) noticed low temperature in the Secondary Combustion Chamber of the Incinerator. So,
    he went to the site for inspection along with his colleague. During site inspection, they suspected clogging in
    Coke Oven Gas (COG) inlet strainer. They opened the strainer after isolating the COG inlet and outlet control
    valve. While opening the spool piece of multiblock, sudden flame/hot gas came out from COG pipeline,
    resulting in burn injuries to Mr. Pradip Mandal. After administering first aid, he was taken to TMH by doctor on
    duty. He was admitted to Burn Care Unit at Tata Main Hospital (TMH). Despite the best efforts put in by the
    team of doctors, he succumbed to injuries on 5th March 2023 at 3:30 AM at TMH.

    Preliminary Findings:
    1. Standard Operating Procedure (SOP) was available for the job.
    2. Mr. Pradip Mandal was engaged in Incinerator Plant operation since 2016.

    Immediate actions taken:


    1. Incident information was communicated to all concerned employees.

    2. PIR (Preliminary Incident Report) was logged in Ensafe system, and the investigation team has
    been formed for detailed investigation.

    Family Details:
    Mr. Pradip Mandal is survived by his father, mother, wife, and a daughter. All assistance, as per company
    norms, is being extended to the family.
