Health & Safety Safety Alert Orange Stripe: 97/FY-24
What happened:
On January 03, 2024, IP (Injured Person) experienced pain in his right knee while ascending the
cage after completing his duty. IP was known to have Osteoarthritis in his legs. The following day,
he sought medical attention due to swollen knee. According to the doctor’s report, he had sprained
his knee, leading to hospital admission for treatment. Consequently, he was unable to resume duty.
Onsetter
Preliminary Findings:
1. IP was suffering from osteoarthritis and walked with limp.
Recommendations:
1. All employees, who have medical issues that could lead to untoward consequences, should
be identified, and appropriate action should be taken to prevent such incidents.
2. Communication should be given to all such employees not to enter the cage during rush hours
and wait for the next cage.
3. The possibilities of maintaining a better arrangement for the waiting line near the entry area
for the cage should be looked into.

What happened:
On January 13, 2024, IP (Injured Person) was assigned duty at Kalapani Pumphouse, Tailing Pond.
IP was locking the operator room after completing his B shift duty. While getting down from the
operator room, his right leg got twisted on the first step of the stair. He was taken to Tata Central
Hospital West Bokaro, where his X-ray report revealed fracture in his right leg ankle.
Preliminary Findings:
1. The absence of a landing platform made it unergonomic and unsafe to turn around properly,
while standing on the staircase after latching the gate.
2. Handrails on both sides of the staircase were missing.
3. Illumination of the site was found sufficient.
Recommendations:
1. The gate/entrance/approach to any facility should be supported with a landing platform of
dimensions at least 1000mm*1000mm in front of it, when approached through a staircase.
2. The staircase, attached to the landing platform, should be made perpendicular to the direction
of approach of the gate with standard handrails. The latching of the gate should not be done
towards the staircase side.

What happened:
On January 25, 2024, IP (Injured Person) was engaged in the auto ball fill machine. During the
process, there was a ball filled bearing coming back along with the jaw of the vice unit. In order to
remove that bearing, IP used bare hands on the machine in the running condition (instead of
stopping the machine and using appropriate tools to do so). Due to the forward motion of the jaw,
his finger got pressed against the machine edge, and he sustained a cut injury to his right index
finger. IP was taken to the first aid centre and then, sent to Spandan Hospital, Medinapore for further
treatment.
Preliminary Findings:
1. Machine was in motion during the time of the incident.
2. Tackle for removing the bearing from that workstation was available.
3. Sufficient illumination was available at workstation.
4. SOP (Standard Operating Procedure) was not followed.
Recommendations:
1. One must use appropriate tool while working on the moving machinery and positively isolate
the energy sources before starting the job.
2. Standard Operating Procedure should be revised, and training on the revised version
should be provided to all employees.

What happened:
On January 27, 2024, IP (Injured Person) along with his co-worker, was involved in a concrete debris
cleaning job at SWPS 29 pit. One person inside the pit was gathering all the debris in a bucket, while
the IP was positioned on the top of the pit slab to lift the debris bucket using a rope. Suddenly, he
lost his balance and fell into approximately 5-meter-deep pit. IP was taken to the Occupational Health
Centre. Later, he was referred to Tata Medica Hospital, where X -ray revealed a fracture in his right
hand near the wrist.
Preliminary Findings:
1. Work permit was taken for the job.
2. SOP (Standard Operating Procedure) was available for the job.
1. IP was taken to first aid centre, later he was referred to Tata Medica Hospital for further
treatment.
2. Incident was communicated to all concerned.
3. PIR (IN00182913) was logged in Ensafe for further investigation.
Recommendations:
1. Temporary barricading around the pit and, proper arrangement for extracting the debris
during such activities should be explored.
2. Unwanted openings should be inspected and covered at any location.
