What Happened?

On 24th March 2023, at around 2:00 PM, deceased person (DP), Mr. Biswajit Bera, CL. No.- 107504 of M/S Jana
Enterprise along with co-worker Mr. Irshad Ali went to the top of Ductile Iron Pipe Plant #1 shed for Inspection and
Assessment of sheet health for future replacement planning. After finishing the job, at around 4PM they started to
walk back. The deceased person stepped over a transparent sheet which resulted in a fall from a height of approx. 16.5
meter over the Annealing Furnace Top. The DP was immediately taken to the local Government Hospital where he was
Declared Brought dead.

Preliminary Findings:
• Deceased person was engaged with M/s Jana Enterprise for Roof Sheeting since 2021
• Standard Operating Procedure (SOP) was available for the job.
• Permit to Work is available.
• Tool Box Talk was conducted.
• Both were using the required PPEs but DP had not anchored it with life line.
• Roof Shed is equipped with Permanent Horizontal Fixed Lifeline System.
• There was a Plan to repair the Roof Shed because of water leakage during rains.

Immediate actions taken:

• All activities related to the Rooftop area were suspended across the Organization.
• Incident site was barricaded to restrict entry.
• All officials and local administration were informed about the Incident. Inspector of Factories visited the site
and completed his Preliminary Investigation.
• Incident information was communicated to all employees.
• Incident (IR/009989) Report logged in One Technology Portal (OTP)
• Incident Investigation team is being constituted.
• Family of DP was informed, and initial support was provided.

Family Details:
Biswajit Bera, 24 year old. DP is survived by his father, Mother and Brother.
All assistance as per company norms, including Suraksha Scheme, will be extended to the family.
