Zuoquan Power plant Accident cement plate form in the middle of the ash silo ( with a vertical drop of
Outsourced person responsible for cleaning and sanitation was
performing cleaning on 2nd floor of T2 transfer station.
He violated safety regulation by crossing the lifting port guard rail
causing him to fall a height of 6.8m. 63 years old man suffered from
internal injuries and was declared dead.
Direct Causes:
Crossing of guard rail was safety violation and Minor collapse of lifting
port guardrail led to his fall and death
Indirect Causes:
1) Inadequate hazard inspection: Company previously performed
hazardous inspection but missed blind spots
2) inadequate safety training:
Personnel did not receive proper safety education
Fuzhou power plant
Accident: unauthorized entry in ESP by outsourced personal fell into the
hopper and died. There was no work ticket and safety measures, no
supervisor for confined space.

some point and han died.

Zhengning Power Plant of Gansu Company
Accident:
Excavation work for waste water pipeline, failure to follow company
guidelines on more than 3m of excavation depth, sides of trench
collapsed, person buried under soil.
Direct Causes:
• Excavation project was not included in the management of hazardous
sub projects
• Inadequate identification of risks
• Supervision of operations not in place
• Insufficient safety measures
Indirect Causes:
• Insufficient on-site daily safety Supervision management
• Incapable safety supervisors
• Insufficient implementation of two ticket systems
• Insufficient safety training of workers
Death accident in Yangluo Power Plant
Accident process:
On march 18, 2023 at 08:05 AM, two members of the work team Mr. Lin
and Mr. Sun, entered the construction site without notice to the power
plant maintenance department. This occurred despite the power plant
requirement to suspend work on rainy days which was the case on
March 17.they engaged in rust removal work on the separator of the 3-
4-3 fly ash silo in phase III of the power plant.
At 11:25 AM, Mr Lin voluntarily removed his safety protections
( including on safety harness) and jumped from the work pipe.
Unfortunately the plate form patterned steel plate ruptured and
perforated ( 800mm × 580m ) as a result Mr. Lin fell down to the
approximately 6.4 m )
Upon receiving the report , project manager Mr. Zhu arrived at the
scene at 11:42 AM. He along with Mr. Sun and the driver, managed to
remove Mr. Lin from the platform location at 11:50AM .the project
manager failed to promptly the incident to the power plant. Instead he
used his own vehicle to transport Mr. Lin to the Yangluo Branch Of
Zhongshan Hospital in hubei province for the emergency treatment.
Sadly, on march 19,2023 at 10:03 AM , Mr. Lin passed away despite the
efforts made to save him.
Direct causes:
1) steel structural platform had corroded
2) indicating insufficient strength
3) Mr. Lin had weak awareness of safety
4) inadequate understanding of the danger associated
Indirect causes:
1) inadequate risk identification
2) hidden danger of rust and even breakage in several areas
3) some stairs and platform railings lacked kickboards.
4) plant did not take temporary preventive measures.
5) weak work organization, personal entered the work without work
permit
6) in charge of the work was not present at the work site.
Prevention measures:
1) organize safety learning sessions
2) enforcement of the two permit system
3) in this lesson to strengthen safety awareness for departmental staff
4) immediately rectify safety hazards such as corroded platforms
5) implement temporary preventive measures.

7) organize inspection for the corrosion status platforms, including steel
structures.
Power Plant name : Huaneng Huajialing Wind Yard
Accident Process: There was a task of measurement of dimensions &
installation verification of “Inverter control transformer” in #21 Wind
turbine yard.
Two persons,Mr. Zhang and Shang(who were responsible for the annual
maintenance of #21 wind turbine) took all the safety measures of
Transformer isolation, opened all the breakers and confirmed the
absence of voltage at the front door of inverter grid cabinet. As they
came out of the yard, they heard an explosion, upon returning they
found out that Mr. Chen died.
Direct Causes: 1- Inverter main Circuit Breaker still had voltage even
though the breaker was opened.
2- Internal inspection of control cabinet had blind spots and
energisation was not identified. The reluctance of low voltage side was
not investigated.
3- The inverter was at a height of 1.7m , hydraulic station was 40 cm in
height and there was unstable stable standing position to inspect
inverter transformer
Exposed Problems:
1- Employees entered the tower base without obtaining work permits
and with inadequate on-site safety supervision.
2- Inadequate Management of Outside Suppliers: Outside personnel
were allowed on site with just a phone call contact and he was not
accompanied by maintenance personnel of wind turbine
3- Incomplete Hazard Inspection: There were blind spots in the internal
inspection of inverter control cabinet and live parts were not thoroughly
investigated.
Preventive measures:
1- Organize Safety Learning from Accident: Strength employees safety
awareness, enhance management of “two permits”
2- Enhance outdoor management: Prohibit external personnel from
entering the maintenance site alone. External personnel can work under
the supervision & guardianship of power plant personnel
3- Enhance Hazard Identification: Provide additional safety awareness
training to improve on-site operational risk identification
4- Strengthen Hazard Inspection & Control: Conduct comprehensive
inspection of low voltage switches and potential reluctance issues.
