Subject Random HSE Inspections Report from 15– 18 Oct. 2016

S/N Observations: Risk


Rating

1 Area1, 4 & 7: M

- Working MEWP’s found holds plant certification of 1 year contrary to QCS


2014 local regulation.
- No exclusion zone below the working spider man riding basket (MEWP).
Action Taken.
- Hot work station in poor condition + found out permit to work is filled out with
wrong information, no spot check being carried out + no fire watcher
present. Action Taken.

Remedy:

- JV to contact the MEWP supplier and present the local regulation stipulated
on QSC 2014. They need to re-test the plant which holds 1 year validity to
conform to the legal requirement.
- JV to ensure before commencing MEWP into operation – working radius
shall be demarcated with orange mesh or physical barrier – to prevent any
unauthorized access.
- JV to re-train person how fill out the hot work correctly + no hot work will be
issue unless physical inspection is being done upon satisfactory by the
issuer – safe system in place.

2 Area 2A, 5A: M

- Permanent protection – cables and wires on floor.


- Unsafe use of scaffolding tower observed.
- Housekeeping issue found blocking off the staircase with mixed waste.
Action Taken.

HSE Findings

Remedy:

- JV Engineers/supervisor and others has a duty of care to protect permanent


finishes from getting damage.
- JV to re-instate the tower to its working standard or use podium to low level
ceiling.
- JV to make clear all access staircases in any way and all times.

3. Area 7A & 8A. M

- 3 no. operatives working unsafely in related to cable pulling e.g. low level
mobile tower + no induction and supervision.
- Working on a ladder unsafely.
- Housekeeping at the TPS 6 corridor.

Remedy:
- JV had temporary stop the activity and send the operatives into induction
room. Investigate who put them into work (supervisor) and impose
appropriate violation as necessary.
- JV to use mobile tower as working platform instead of ladder – which should
only be use as short duration and access.
- JV to carry out a sensible housekeeping need attention on corridors.
4. Area 2, 5 and Phase 1 logistic/External: M

- Housekeeping issue e.g. formworks at hoarding fence + redundant material


along way of parapet MSCP roof.
- Travelling cherry picker (MEWP) on a busy area mixed up with pedestrians
without present of flagman.
- Operatives were unsafely positioned closed to operational breaking
excavator at risk of ejecting debris/stuck by machine.
- Suspended load over the team of rebar ignoring the whistle of rigger.
- Missing edge protection in some area of slab retention tank. Action Taken.
- Reversing water lorry into restricted area without present of flagman.
- Spider MEWP without exclusion zone + untidy workplace.
- Roof staircases to update signage blocking access and alternative
directional arrows as these staircases are primarily planned as escape
routes. Action Taken.

Remedy:

- JV to clear off materials at parapet wall roof to allow wider space for traffic.
- JV to ensure no operator shall travel alone without the assistance of flagman
guiding vehicle especially on mixed pedestrians or constraint location.
- JV to conduct safety stand down to the team ref. to men machine interface.
- JV to re-brief the steel fixer group not to ignore rigger instructions/signal
during lifting – never work or pass under the load.
- JV to coordinate with the logistic team in related to closure of staircases and
helps to plan alternative routes putting directional signage.

5 Area 3A, 4A: M

- Damaged and delaminated ply woods covering/protection under high


voltage cables.
- Urinals cubicles provided at roof not being maintained cleaning and
disinfectants.
- Redundant materials + trailing extension leads. Action Taken.
- Missing exclusion zone below block laying wall. Action Taken.
- Unsafe use of low level railing mobile scaffold. Action Taken.

Remedy:
- JV to replace protection of cable with more durable materials and made of
fire hazard materials.
- JV to assign person dedicated to clean and disinfectant temporary urinals.
- JV to remove unwanted materials at undercroft pocket areas as the building
on the finishing stage.
- JV to install exclusion zone below the block laying.

6 Area 3 & 6: M
- Unidentified cables laid over ground unprotected + no warning signs.
- Improvised cutting knife is used by operative + himself using unsafe and
untagged mobile tower.
- Unauthorized toilet found at BOH corridor.
- Planned stone work closing balcony with no coordination to safety/logistic
failing to provide alternative access directional arrows at 1st level floor.
- Housekeeping issues observed.
- Unguarded abrasive wheel presumed it was used by operative.

Remedy:

- JV to call out concern person/electrician to trace the cables be protected


and put out warning signage.
- JV to confiscate and condemn any makeshift equipment or devices + need
to communicated to team during DSTI.
- JV to apply lime power to clean removing foul-odor. Security personnel
assigned on mall shall continuously patrolling esp. at BOH and staircases.
Anyone will be caught shall immediately be terminated from site.
- JV to establish coordination to stone laying team before closing section of
floor/s to develop plans providing alternative access and arrow signage.
- JV to randomly check power tools that was being used + end user never to
modified or made alteration if not authorized to do so. Never operate rotating
machine without guards.

7. Area 1A, 6A and 8: M


- Unsafe working on mobile scaffold + poor in lighting and supervision during
coring concrete.
- Arrangement of cables required.
- Missing door on energized TPS 7 room – JV to speed up the installation of
permanent door.
- Protection of equipment – dust escalator.
- Used of extremely unstable /narrow mobile scaffold.
- Loose /unsecured edge protection on floor edge balustrade glass + warning
signs.
- Empty wooden stillage + waste and unwanted materials noted in several
areas.

Remedy:

- JV must enforce a close supervision to certain activity which is classified as


high risk job to ensure safe method of work is being followed.
- JV to improve cable management site wide by routing cables
overhead/regular checks of temporary cables condition.
- JV Engineer /supervisor/workers have the duty of care to protect permanent
finishes – escalator must be covered with blue sheeting.
- JV to use suitable mobile tower – narrow structure is permitted to use on
toilet areas which with limited space.
- JV to replace loose combi safe with wooden board and secured and posting
warning signs.
- JV to make sure daily removal of wooden stillage was this identified as fire
hazards + redundant materials in pocket areas.

8 Area Phase 2 Logistic and External: M


- Construction waste observed e.g. striking formworks, tunnel cable pulling,
mountain-like steel cut outs, and arrangement of VAFE temporary staging
materials.
- Reversing grader machine without the present of flagman.

Remedy:
- JV to start cleaning /clearing waste and redundant materials, expedite the
collection of steel cut-outs to contractor.
- JV to make sure engineer/supervisor in-charge shall arrange or allocate
flagman to plant during operation at all times.
