Unsafe lifting gear:

RPCO:

The safety lock on the chain sling’s hook used for lifting GRPE pipes is
missing. This makes the hook/latch unreliable as it could easily open,
causing a sudden loss of load during lifting.

Category: (Lifting).

Recommendations:

The missing safety lock to be provided immediately, inspection and


color coding of the chain sling should also be done. Third party
inspection for all lifting gears is required

Excavation without barricades:

RPCO:

There are still a number of deep excavations on site without proper


barricades or soil bunds.

Category: (Excavation).

Recommendations:

Immediate action is required by RPCO to provide barricades along all


edges of the such deep and unprotected excavations to prevent any
foreseeable accident on site.

Improper storge of oil drums:

BGA Crush plant:

Oil drums are being placed under direct sunlight with no spill
protection.

Category: (Oil/Chemical handling).

Recommendations:

All oil and chemical drums should be stored in properly designed


chemical storage areas with drip trays as well as shading on top so
that they are kept away from direct sunlight and from any ignition
sources. SDSs should also be provided for each chemical container.

Concrete waste at site:

RPCO:

Accumulated concrete waste seen in several areas at site.

Category: (Waste management).

Recommendations:

Clean up all debris and concrete waste from the site ASAP to
designated area in compliance with the site’s waste management
procedure.

Improper storage of truck tires:

Pile of truck tires noticed at IKK asphalt plant.

Category: (Material storage).

Recommendations:

Remove any unnecessary materials from the site and re-use or


recycle them whenever possible.

GOOD PRACTICES:

1. A designated hot work area is provided by TMC away from ignition sources and flamable materials.
2. Monitoring the temperature of persons entering the site by security personnel.
3. RPCO sprinkling water on the gatch road as part of dust control efforts.
