Overspeeding tanker:

IKK:

IKK tanker driver was speeding over the limit on the gatch-road
creating excessive dust and reduced visibility for other road users
which could potentially result in an accident.

Category: (Traffic violation).

Recommendations:

Issue warning to the driver and ensure that all drivers have been
instructed on the traffic rules within the site premises

Improper storage of pressurized gas cylinders:

BGA Crush plant:

There is no designated area for storing acetylene cylinders at BGA


crush plant, so cylinders are stored sideways on the ground under
direct sunlight. This can result in the cylinder heating up and
exploding.

Category: (Heavy equipoment).

Recommendations:

Provide a shaded storage area for pressurized gas cylinders and


ensure that all the cylinders are secured with safety chains or straps.

GOOD PRACTICES:

1. Tool box talk was conducted for truck and tanker drivers about wearing seat belts, not using mobile phones
while driving and following speed limits within the site premises.
