Injury : Avulsion injury over tip of right-hand middle finger

What happened:

On June 13, 2023, IP (Injured Person) observed that sparks were getting generated due to rubbing
of stock with the inner bore of troughs after NTM Breakout Box (BOB). To address the abnormality,
he decided to inspect the area by stopping the mill. For inspecting BOB and trough, the upper part
of the BOB was lifted, and alignment rod was inserted into the bore of trough. During removal of
alignment rod, the upper portion of BOB got closed by gravity as it had not reached its position of
rest. The right-hand middle finger of the injured person was caught between top and bottom part of
the BOB resulting in avulsion injury over tip of right-hand middle finger. IP was taken to EPFA (East
Plant First Aid), thereafter he was referred to TMH.

Preliminary Findings:
1. Standard Operating Procedure for the job was available and was being followed.
2. Illumination was found appropriate.
3. Play in the BOB hinge pin was observed along with accumulation of dirt in area left over by
cooling water.

Immediate actions taken:

1. IP was taken to EPFA; thereafter he was referred to TMH for further treatment.
2. Incident was communicated to all concerned persons.
3. PIR (IN00167152) was logged in Ensafe for further investigation.

Recommendations:

1. Locking arrangement should be provided for breakout box to avoid free fall of its upper
portion.
2. Scheduled cleaning should be done to avoid restrictions in the movement of breakout boxes
due to dirt deposits.
