INCIDENT REPORT

B. DESCRIPTION OF THE INCIDENT :

Date : 28 Januari 2010


Time : 3.59 p.m
Location : Manufacturing section, 3rd floor
Equipment involved : Iron bar cutting machine
Injury : Left feet
Details :

Jack Sparro was doing his routine check on the iron bar cutting machine. He saw
one of the bolt that holds the cutting plate was broken. So he decided to change it. As the
bolt’s position was under the machine, he used an hoist to lift the machine up. Jack
Sparro started to change the bolt when the hoist’s chain that was holding the machine
broke suddenly. The machine fell right to his left feet. His feet was seriously injured. No
one heard his yell as the section was noisy with operating machines. However, Chaey
Mun and Aung Sim who came in with intention to shut down the machine later saw Jack
was lying helplessly in the floor. On seeing Jack’s injury, Chaey quickly ran to get first
aid and called the ambulance. While Aung immediately used another hoist to lift off the
machine from Jack’s feet.
The ambulance arrived sharply at 4.30 minutes. Jack was taken to the Damai
Hospital. Dr. Azmee examined Jack’s feet and did an X – ray. He found out that all the
fingers and ankle on Jack’s left feet were fractured and needed to be operated. On the
next day, an operation was carried out. Jack was advised to remain hospitalized for at
least three weeks.

C. CAUSES :

After an initial investigation, I discovered that step 34 of work procedure no.6 was not
followed. The work requires any in charge technician to conduct an monthly safety
maintenance on the hoist. As the technician who was in charge to maintain the hoist was
on leave, the hoist was left unmaintained. As a result, no one was informed that the hoist
was not safe to be used. A further investigation revealed that no BREAKDOWN
EQUIPMENT sticker was attached to the hoist.

D. RECOMMENDATION :

To prevent similar occurrences. I suggest the following :

1) A refresher course on safety procedure should be conducted.


2) Any equipment failure should be reported immediately.
