What happened:

M/s Keller Ground Engineering India Private Limited is engaged for ground improvement & piling job at EAF Ludhiana project.
IP (Bhupinder Kumar) was engaged in operation of feeding conveyor in batching plant.

At Around 05:10 AM, I.P was going to see the safe movement of JCB, which was engaged to feeding aggregate in batching plant
hopper. While going IP got slipped and fell on stone pebbles on ground. Which lead to injury in right hand arm. Immediately IP
was taken to Fortis Hospital, Ludhiana, where doctor advise for X-Ray. Fracture of Right-hand arm detected in X-Ray. I.P Admit
in Fortis Hospital on 16th Dec 23 and discharged on 17th Dec 23.

Preliminary findings:

1. Work permit was available.

2. Toolbox talk was given to the workmen before work started.
3. IP was using all required PPEs.

1. IP was immediately taken to Fortis Hospital, Ludhiana for treatment.
2. Incident was communicated to all concerned employees.
3. Incident was logged in Ensafe (PIR – IN00179813) system for detailed investigation.
