Injury : Swelling on left ankle.
What happened:
On 13th September 2021, the injured person (IP) was in ‘C’ shift duty. He went to inspect the Euclid Dumper
No - 137 which was parked near LD#3 scrap dumping area. After inspection, while he was getting down
from the dumper, he became unbalanced resulting in an injury to his left ankle. IP was immediately taken to
the East Plant First Aid Centre and therefrom referred to TMH for further treatment.

Preliminary Findings:
1. IP was following the three-point contacts rule while getting down from the vehicle.
2. Floor condition at the incident site was found unsafe.
3. Illumination at the incident site was found adequate.

Immediate actions taken:


1. Incident was communicated to all concerned employees.
2. Incident was logged in ENSAFE (PIR - IN00124126) system for detailed investigation.

Recommendations:
1. Inspection of the vehicle should be conducted at the designated place of the customer site.
2. All employees should be attentive while getting up/down from the vehicle.
