Describe briefly how the incident occurred (Add sketches and additional sheets to support
the description):

During the shifting of Reinforcement rod from material yard to Cement silo location, the trailer
driver hits the cable pole while reversing the trailer. Due to this incident no one got injured and
there was a small damage in the cable pole.

 Driver Negligence

Root Causes:
 Lack of Supervision
 Absence of flagmen
