Brief Incident Description

A las 2:35 h, el operador del tractor de una empresa

contratista, realizaba la limpieza de la cuneta en la Rampa
387 del Nivel 1070, en ese momento, la cadena de la oruga
queda atrapada en el lodo y al intentar traccionar, el equipo
impacta la estructura posterior izquierda de la cabina contra
una alcayata del hastial.

No se reportaron lesiones personales.

Consequences
• Raspadura de cinta reflectiva y estructura posterior izquierdo de la cabina.

Immediate Actions
• El supervisor de operaciones reporta el evento a Central de Emergencias.
• El trabajador pasa prueba de alcoholemia con resultado negativo.
• Se inicia el proceso de investigación con el equipo multidisciplinario.
