1. OBJECTIVE

To define the information to be communicated to the leading unit and analysed at site in case
of an incident at site within the scope of Smart Energy EPC projects and processes.

3. RESPONSIBILITY

Project manager, Site chief, Storage officer, Site engineer, EHS officer, Field technician,
Foreman, Supervisor, Quality Control Officer are responsible for the follow-up or support the overall
process where applicable.

5. METHOD

For all incidents (major accident, lost time accident, accident without lost time, first aid, near
miss):

The Health officer (EHS Officer) shall record all the accidents, incidents and near misses with
the following information and that for the Smart Energy staff but also for our subcontractor
at site.

• Description of the incident (including who is involved and the date)

• The number of hours / days lost
• The direct cost
• The indirect cost (where applicable)
• The root cause and the action to be implemented to avoid a second incident
• The closing action date

Corrective action shall be validated by the EHS Responsible

In case of major incident:

Refer to SB Major incident procedure

In case of Lost time accident:

Accident declaration report should be filled in and passed by mail within 24 hours following
the accident to the concerned site activities department.

6. INSPECTION & CONTROL

Site Chief / Manager & EHS Responsible is responsible for ;

 Declare that incidents are declared and analysed as per requirement.

 Analysis & evaluation of historical data should be done and reported for each project
as a reference for lessons learned database and future projects, where applicable.
