What happened?

Mr Loto Mahakur (Excavator Driver) aged 30 years of M/s R R EQUICONS PRIVATE LIMITED was deputed
to unload excavator from trailor. While unloading of excavator from the trailor, excavator tilted towards
left side and toppled on road. When excavator started tilting, the excavator driver came out from
operator cabin. During this incident, no injury happened. Excavator driver cabin damaged.

Preliminary Findings:
1. M/S R R Equicons were executing unloading of excavator from trailor without informing to
department.
2. Unloading carried out at a place where level difference between trailer bed and ground
surface (1.2m).
3. Unloading of the excavator was being done through boom of the excavator instead of ramp.
4. Work permit not taken by the Vendor for unloading of excavator job. SOP was not available.
Area was not barricaded during unloading of excavator. Flagman was not available during
unloading.
5. Driver entered the plant with a visitor gate pass approved by Head (Engg & Proj.) for the
purpose of meeting, but he was engaged in unloading of the excavator.
6. Driver was having valid licence for LDRXCV (Loader, Excavator, Hydraulic Equipment).

Immediate action:
1. Job stopped immediately & area barricading done. The excavator was restored back with
the help of 2XF15 Crane.

2. Reported incident to safety department, other senior executives and all concerned.
3. Incident logged in Ensafe. – IN00155238. & Cross functional Enquiry Committee formed
including expert from TSJ to investigate the incident.

4. Detailed investigation is in progress.
