Incident Description- During lifting and shifting of Puff panel at around 15:00 on date 01-04-2023,the panel
rack containing around 10 panel and weight approx 800kg is being fall down due to improper hitch of web
sling,lack of supervision,uncompeent operator of Hydra.
During lifting of Puff panel,the web sling hitch get sliped from position due to puff panel tilted at access floor
and fall down.

Root cause of Accident:1.Incompetency of Rigging Knowledge
2.Lack of Supervision
3.No Proper Communication while rigging
4. No Barrication

What could have done to prevent this injury/Near miss:Provide Safe Rigging training about hitch,hydra
radius,boom length,counter weight to prevent toppling ,suspension load consequences hazard to all
supervisiors and workers involved.Infuture competent riggerd involved in lifting, shifting Activity.

Incident Description- During steam testing charging of WTP media testing of 4 inch steam line tesing wo
suddenly pressure guage flies from his connection thread point due to loose fiting and not used exact
materials of pressure guage siphon .Pressure gauge landed no man place so that no injury happened .

to IBR siphon and checked all thraed joint and flange connection properly to prevent like this event in
future.Given all relatived worker training and tell all consequences about this evnet .
