    1) PWHT materials were found in scattered condition on the scaffolding platform of 4016 new module.
    2) Insulation Material not proper stacking.
    3) Plastic gunny bags were used for carry glass wool used for PWHT work.
    4) PWHT cables were found in scattered condition in walkway & creating tripping hazards throughout the
    grating Platform on the reactor area and tame yard 4106 area.
