What happened
While the forklift was performing a reverse maneuver, it tapped on one side of
the tubing causing the other side to fall to the ground and strike the
derrickman’s right foot fracturing the ankle’s joint.
Initial Findings
The worksite area was narrow for the forklift to move freely;
The forklift operator was performing maneuver without signals;
There was no adequate supervision at the time of the incident;
Initial learning
Forklift operations shall always be supported with signals;
Banksman shall ALWAYS be used to ensure safety while working with mobile equipment
Worksite supervisor shall always ensure that control measures are in place prior to authorizing the work

What happened
While attempting to pull a snagged line in the crane sheave block a second
crane operator’s hand was trapped when the block dropped suddenly injuring
his finger.

Initial learning
Always keep your hands/fingers away from crush points and ensure understanding the risk of stored energy;
Always follow proper maintenance and trouble shooting procedures.
When intervened for unsafe act, all personnel are obliged to STOP.

What happened
Chief Electrician left index finger got trapped between the cable
cover and the ground While trying to insert the electrical cable
cover chains into the forks to flip the cover around the cables.
Chief Electrician place his hand in the line of fire ( crushing point)
