place at a Newmont operation. The information below is a preliminary assessment and not a formal investigation. Please
complete within 72hrs of the original event, and then forward to Corporate HSLP for review and global distribution.
LOCATION/DEPARTMENT:
In PIT RL300 Intersection X
Katala Ramp area
At 09:18 am the Operator of HT016 was assigned to SH02 for
loading after dumping material at Tongoloka Ujat RD255. While he
was traveling towards the loading point at SH002 he received an
INCIDENT/ ACCIDENT
instruction from Dispatch to proceed to the Pondok Crusher and not
DESCRIPTION:
to SH02. When he reached the RL 300 intersection near the Katala
Pondok, he turned right without stopping at the junction and collided
with HT053 which was hauling material from the Katala ramp toward
Tongoloka waste dump.
Production loss as the HT has been removed from the operating fleet
POTENTIAL FOR INJURY / LEVEL: 3 Moderate 4 Major 5 Catastrophic
LOSS and DESCRIPTION of
CONSEQUENCE RATING: Potential Fatal Occurrence
This accident could have resulted in a serious injury and or fatality.

INCIDENT / ACCIDENT
DESCRIPTION :
Pada hari kamis tanggal 10 maret 2011 korban dan rekan sejumlah 3 orang
dari Sub Contractor mengikuti safety induction dari petugas Safety Dep’t,
untuk mengerjakan memperbaiki mesin bubut di fabrikasi roll dan pada
dini hari tanggal. 15 Maret 2011 jam. 15.00 ,terjadi Accident,
korban mengoperasikan’ test mesin tangan kiri memegang handel drive
pada posisi O ( Zero ) dihadapan putaran ( Chak ) bagian pengikat benda,
dari chak tersebut grip terlepas dari posisinya dan mengenai mata korban,
korban terluka pada mata dan dilakukan P3K ditempat dilanjutkan pengo
batan keklinik Budi Asih dan dirujuk di Rumah Sakit Sari Asih Tangerang
untuk mendapatkan perawatan Dokter Operasi Mata.
PROBABEL IMMEDIATE
CAUSES :
Kasus kejadian ini akan di tindak lanjuti segera ( Investigasi )

Keterangan : Dari hasil investigasi dan beberapa sumber dilapangan bahwa krono
logis kejadian sebagai berikut: sebelumnya sisa kabel listrik dari mesin
genset Denyo portabel dibentuk memanjang putaran kedalam ruang
genset ( Layer ) karena ada kegiatan konstruksi baja penyangga deck
plafon / perbaikan bangunan genset kabel tersebut dikeluarkan dan
digulung bentuk lingkaran,

pengawalan Atau assistant, pe mbantu supir, sedangkan schedule perJalanan jauh
dan di harap kan bisa tercapai SELAMAT KE TUJUAN, .kondisi kendaraan truck
laik pakai dan terawat / terpeli hara jurumudi ber pengalaman kenapa kecelakaan

REKOMENDASI Faktor kebiasan yang berpotensi resiko tidak selamanya baik selalu
dilakukan sehingga lakukanlah cara kerja aman yang tidak berpotensi
bahaya, bekerjalah sesuai prosedur, pakailah alat pelindung diri sesuai
kan dengan pekerjaan,
Bekerjalah dengan hati yang senang, dan tidak panik atau terburu buru
dilarang bekerja dengan perasaan merasa ditekan, bekerjalah secara
proporsional, untuk mencapai keselamatan produksi yang optimal,aman
bertanyalah atau sampaikanlah pada atasan bila ada keraguan yang ber
potensi beresiko,
