Section A (Description of event / potential hazard)
Date : 27 December 2020
Ship’s position = 19° 55,103 N / 058° 04,811 E

When we checked Main Air Compressor No.1 on running, 3rd Engineer reported that the Cylinder Head due
to leakage.

Potential Hazards
- Main Air Compressor continue running and can over heat.
- Can not fill up Air Reservoir.

(1) Investigation of incident (primary and / or contributing causes)
a. Lack of maintenance (due to Main Air Compressor auto running)

(2) Corrective Action taken (remove hazard, replace, repair or re-train in proper procedures for the
task)
a. Change over running from No.1 to No.2 Main Air Compressor.
b. Blank Electric power and open up cylinder head for check packing.
c. Changed with new spare this packing so cleaned and lapped low pressure valve.
d. Replaced again all parts.
e. Running test and satisfactory.

(3) Preventive Action (Action to prevent the same matter will not recurrent)
a. To be check condition of all equipment before use and make sure it is safe.
b. Immediately report to chief engineer if found any defect of machinery & equipment
c. Spare parts must be have on board.
